We welcome referrals from health and social care professionals for individuals requiring supported living or domiciliary care.
Our assessment process ensures each person receives the right level of care in the most appropriate setting.

Submit Referral
Professionals can submit a referral with supporting documentation (see checklist below).

Assessment
Our team carries out a comprehensive needs assessment, including clinical input and risk review.

Admission & Support Plan
Once approved, a personalised care and support plan is created in partnership with the individual.

Full risk assessment

Copies of recent CPA meetings

Mental Health Act status (and expected changes)

Contact details for responsible parties and emergency contacts

Conditional discharge details (if applicable)

Information on family, friends, or support network

Assessment of daily living skills

Overview of strengths, needs, preferences, and activities

Confirmation of funding and trial period details

Each referral includes a trial period of up to six weeks to ensure the placement is suitable and meets the individual’s needs. Written confirmation of funding from the responsible authority must be received before admission or trial leave begins. A thorough assessment will be completed before final admission.
